| The
adult programs of The Center provide a wide array of habilitative
supports and services to individuals over the age of twenty-one.
The adult division includes the residential, day treatment, supported
employment, in-home waiver, and individual support services
programs. Additional services include clinic, medical/nursing, and
service coordination.
The Adult Service staff uses
a person centered approach to help individuals we serve realize dreams
about living, working and recreating in communities of their
choice. By empowering people, forming partnerships, and supporting
choices, we work to achieve our goal of supporting people as they experience
self-advocacy and meaningful inclusion in community living.
Accomplishments:
- Initiated strategic planning specific to housing
development. In the past year, eight new residential
opportunities were created in a variety of apartment and small
homelike settings. As a result, eight individuals moved from
the residential waitlist into appropriate residential options.
In addition, the creation of backfills allowed twenty-one
individuals to move to more appropriate residential opportunities of
their choice.
- Participated in lobbying efforts in concert with
the alliance of Long Island Agencies (ALIA) and New York State
Association of Community and Residential Agencies (NYSACRA) to
successfully raise capital thresholds for new home acquisition on
Long Island.
- Received the excellence award for Grassroots
Advocacy given annually by NYSACRA. The award recognizes
outstanding effort and achievement in advocacy and lobbying
activities.
- Received approval from the Commissioner of State
Education and Board of Regents to occupy a seat on the New York
State Rehabilitation Council.
- Completed phase III of person centered planning
sponsored by the NYS Office of Mental Retardation and Developmental
Disabilities (OMRDD) in conjunction with Cornell University.
- Presented training on person centered planning and
structured teaching at statewide conferences sponsored by the
NYSACRA and the New York state Association of Day Service Providers.
- Successfully established the use of internships
with Adelphi University and Hofstra University for our speech
department. Initiated phase I of a similar model for our
psychology department.
- Completed the restructuring of nursing services in
line with our philosophy that consumer driven services are the
responsibility of program directors.
- Developed quality environment committees for all
Adult Service Program sites to foster partnerships in meeting
site-specific goals.
