Adult Day Services
The Adult Day Service Division is based out of the Center's Hicksville Campus and provides three basic service modalities: the Adult Day Treatment Program, the Adult Supported Employment Program, and two distinct Adult Day Habilitation Programs.
The paramount goal of all our services is to provide the training and supports necessary to help each individual achieve their maximum level of autonomy and empowerment. Each program model has been designed with this goal in mind and is certified under the auspices of New York State Office of Mental Retardation and Developmental Disabilities and/or Office of Vocational and Educational Services for Individuals with Disabilities.
Adult Day Treatment
The Day Treatment Program provides programming and professional services to approximately 65 individuals 21 years of age and older with developmental disabilities. The program provides a highly structured clinical based environment using the treatment team model to meet the needs of the individuals served. Some of the support services provided include but are not limited to the areas of independent living skills, self care, medical care, nursing services, nutritional counseling, psychology, social work, speech pathology, and therapeutic recreation. The staffing ratio is typically 1 to 4 with direct support staff providing group and individual training in the areas of activity of daily living skills, communication skills, recreational skills and prevocational skills.
Adult Day Habilitation
The Day Habilitation Programs are a Home and Community Bond Services waiver service based on the Person Centered Approach. The day habilitation plan is based on the Individualized Service Plan or Comprehensive Functional Assessment and is written in collaboration with the consumer, service coordinator/Qualified Mental Retardation Professional and advocate. It is centered on the valued outcomes of the individual; their personal interests and aspirations. Day habilitation services help the person learn new skills, have new experiences and get the supports necessary to pursue their valued outcomes. These services are very flexible and can be provided almost anywhere. Day Habilitation is a compilation of supports and services rooted in an individual's likes and desires. The Center currently provides Day Habilitation services to over 80 adults with developmental disabilities through two distinct programs.
Building Bridges
Our Building Bridges Program was opened in December of 2000 and is a day habilitation program geared towards individuals who are seeking an active community based program focusing on volunteerism and the daily use of community resources. The staffing ratio is typically 1 to 5 with staff providing training in socialization skills, travel training and the use of community resources.
New Beginnings
Our New Beginnings program was officially opened on December 23, 2005 and is a day habilitation program geared towards individuals who are seeking more community involvement, but cannot handle the rigorous schedule of full time daily community activity. Individuals in this program participate in volunteer activities and or utilize community resources as part of their workweek, but also participate in structured activities back at the Hicksville Campus on a daily basis where they have the opportunity to practice needed skills and gain self-confidence prior to going out into the community. The program typically provides a 1 to 4 staff to consumer ratio to allow for small group activities both at the base site and in the community.
Adult Supported Employment
The Adult Supported Employment Program is a vocational community placement and training program serving over 60 adults with developmental disabilities who have demonstrated the skills and desire necessary to work in competitive employment. Its goal is to provide the training supports necessary for these individuals to successfully secure and maintain community based employment, and become a valued member of an employer's stable work force. The program provides; vocational counseling and assessment, job development, on the job training, work place advocacy, sensitivity training to employers and co-workers, and assistance with maintaining social security benefits through various work incentive programs.
